Some of the files in HTML_QuickForm-3.2.4pl1/QuickForm, don't have read permissions for others, and that cause this error:

If you installed the files manually then you need to fix permissions yourself. If PEAR installer installed the files with wrong permissions then open a bug report for PEAR installer.
