I did an upgrade from 0.5.0 to 0.5.1 and got this error message:
Undefined index: username in /usr/share/php/HTML/QuickForm/Renderer/Tableless.php at line 441.
The error occured only in one of my forms, the one without a header element. It is easy to fix with replacing the != '' check against a check with !empty().

Thank you for your bug report. This issue has been fixed
in the latest released version of the package, which you can download at
http://pear.php.net/get/HTML_QuickForm_Renderer_Tableless

I've used another fix, similar to the condition a few lines above.
