Split converters out of PhpDocumentor, to become their own PEAR packages.

Other small enhancement may be included.

Considering increasing the minimum PHP version requirement to v4.3.0, to allow removal of old non-tokenizer internals.

Informal list of ideas:
- Moving to a build system for non pear build so we can actually rely on pear packages
- If possible moving to a pear package for parsing command line options
- consider using php_compat when dealing with PHP version differences
- find a pear package to handle file i/o itself (need to make -fdti arg values easier to use)
- Creating an Public API for using PhpDocumentor and moving the CLI and web front ends to using it
    (all the directory listing stuff would be in a utility library not in the public api)
- Refactoring the docbook code, moving to something along of lines of XML_Transformer
- Refactor converter code (for 2.0 this shouldn't be a major change just focus on cutting down 
    class size and the huge amounts of copy/paste)
- Create a storage API with an in memory implementation and a pdo implementation
- Refactoring configuration... realign runtime options in GNU-ish style, with long deprecation period to allow other projects
    (like phing) to adjust... manual should encourage config file settings as normal usage 
    and allow commandline override usage, rather than "commandline usage OR config file" one-or-the-other usage
- add extension option that uses PHP_DocBlockGenerator to generate skeleton
    docblocks when documentable elements are undocumented (skeleton docblocks would have 
    @todo WRITE DOCBLOCK TEXT in it)
- improve processing time of "--sourcecode on" option
- add option for tutorials written in multiple languages (see SF Help topic msg_id=4901209)
